This plug-in appears to be in the 7000-series form factor. It could have been a prototype for a plug-in that was never produced. The circuit uses the 155-0056-00 sweep control IC and the 155-0028-00 Miller Integrator and delay pick-off IC. The input attenuator board appears to be from the 323 scope.
